A Network for Better Care Coordination

Our 211OC Partner Network brings together Orange County’s health and human services providers to meet residents’ needs through a whole-person approach to care.

 

For example, a person seeking food assistance may also benefit from housing resources, mental health services, or job support. By addressing multiple aspects of a person’s life simultaneously, we empower individuals to make lasting, meaningful improvements.

How it Works

The 2-1-1 Orange County (211OC) Partner Network is made up of thousands of local health and human service providers throughout Orange County who have access to our innovative Community Information Exchange (CIE). 

 

By sharing information across providers (via our GetHelpOC™ platform), individuals avoid the need to repeatedly retell their stories, which reduces the emotional strain on them and minimizes trauma. For service providers, this streamlined process means more time can be dedicated to effective case management rather than redundant data intake, allowing for more focused, compassionate care.
